ABSTRACT

Objective@#To explore the clinical effect of bulk-fill composite resin combined with transparent preformed crowns for aesthetic restoration of deciduous incisor of children.@*Methods@#A predesigned clinical prospective randomized controlled research method was used to select 90 patients (123 teeth). The random number table method was divided into three groups: A, B, and C. Group A was treated with a bulk-fill composite resin of SF (SonicFill) combined with a transparent preformed crown (41 teeth in 30 cases), and group B was treated with a large block of Tetric N-Ceram Bulk Fill Composite resin combined with transparent preformed crowns (39 teeth in 29 cases). Group C was treated with 3M Z350 XT universal nano resin combined with transparent preformed crowns (43 teeth in 31 cases). The visual analog scale (VAS) and the modified USPHS standard were used to evaluate the completeness, marginal steps, marginal discoloration, surface condition, secondary caries and satisfaction of the parents with prostheses after 12 months.@*Results @#Twelve months after the operation, the evaluation indexes of group A were better than those of group B and group C, and the differences were statistically significant, including edge integrity (χ2=10.847, P=0.028), edge step (χ2=7.799, P=0.020), edge discoloration (χ2=10.391, P=0.034), surface state (χ2=11.476, P=0.021), and secondary caries (χ2=10.447, P=0.034). The satisfaction of parents in group A on the overall contour (χ2=10.238, P=0.037), shape and texture (χ2=11.521, P=0.021) were better than those in group B and group C, and the differences were statistically significant. There was no significant difference in the evaluation of color satisfaction among the three groups (χ2=0.990, P=0.610).@* Conclusion@#SonicFill bulk-fill composite resin combined with transparent preformed crown is good for short-term aesthetic restoration of deciduous incisor, and parental satisfaction is high.

ABSTRACT

Las células cuentan con mecanismos complejos que vigilan la integridad del ADN, activando mecanismos de reparación cuando hay deficiencias o errores durante la replicación. Una consecuencia potencial de los daños son las alteraciones permanentes en la estructura del ADN que pueden generar mutaciones, transformación carcinogénica y muerte celular. Estos son atribuidos a diferentes agentes endógenos como los radicales libres de oxígeno (RLO) provenientes de la respiración, los cuales son considerados el centro de la carcinogénesis y el envejecimiento por daño genómico; agentes exógenos como la luz ultravioleta que inducen dímeros de pirimidina y la radiación ionizante que produce una gran variedad de daños sobre las bases, muchos de ellos por efecto indirecto. También se encuentran las genotoxinas presentes en los alimentos, humo de tabaco y agentes quimioterapéuticos, con grandes cualidades para alterar la estructura de la molécula ADN e interferir con su expresión. De esta manera, cerca de 10(5) lesiones espontáneas por día son inducidas en nuestros genes, en donde los mecanismos de reparación detectan daños y perturbaciones durante el crecimiento y división celular. Esto es posible gracias a las funciones específicas de reconocimiento, corrección o eliminación de daños que asegura la integridad del genoma. En este artículo se presentan los principales mecanismos de reparación del ADN, su relación y activación de acuerdo al tipo de daño.


Cells have complex mechanisms that monitor DNA integrity that activate repair mechanisms when there are deficiencies or errors during replication. A potential result of the damage is a permanent alteration in DNA structure that can generate mutations, carcinogenic transformation and cell death. These are attributed to different endogenous agents such as oxygen free radicals (OFR) from respiration, which are considered the center of carcinogenesis and aging process due to genomic damage; exogenous agents, such as ultraviolet light, induce pyrimidine dimers and ionizing radiation that produce a variety of damage on the bases, many by indirect effect. Genotoxins present in food, tobacco and chemotherapeutic agents are also found with high potential in altering the DNA molecule structure and interfering with its expression. Thus, around 10(5) spontaneous lesions are induced per day in our genes, where the repair mechanisms can detect damages and disturbances during cell growth and division. This is possible thanks to the specific recognition, correction or elimination of damage functions, ensuring the integrity of the genome. In this article the main mechanisms of DNA repair, as well as their relationship and activation according to the type of damage, are presented.

ABSTRACT

We report here on a new arthroscopic direct repair technique for a radial tear of the posterior root of the medial meniscus (PRMM) using a posterior trans-septal portal. Radial tears of the PRMM are commonly observed in the elderly population of Korea and Japan, and the life style of these people requires squatting and kneeling down in daily life. A radial tear of the PRMM results in the loss of hoop tension and this accelerates degenerative changes in the knee joint and causes early osteoarthritis. Several reports in the medical literature have focused on various repair techniques for these tears by using pull out sutures. These techniques result in nonanatomic fixation of the meniscus, which may lead to disturbed meniscal excursion and failure to restore hoop tension. Arthroscopic direct repair may contribute to restoring hoop tension and preventing accelerated degenerative changes in the knee joint of these patients.

ABSTRACT

OBJECTIVE: There were many instruments introduced for direct repair of spondylolysis. But those instruments were not designed for direct repair of spondylolysis originally. Dynesys implants consist of pedicle screws and cords or laminar hooks that were designed for lumbar dynamic stabilization. We used pedicle screws and cord or laminar hook of Dynesys spinal system for repairment of spondylolysis. METHODS: July 2005 to March 2009, Six patients with lumbar spondylolysis were treated with a new technique using a pedicle screw with a u-shaped rod or laminar hook combined with bone grafting of the transverse process and lamina of the same level, bypassing the spondylolytic defect. There were 5 males and 1 female. Ages ranged from 18-48 years(average of 34.5 years). A total of 8 vertebral levels were fused(one patient had spondylolytic defects at 3 levels). Five defects were at L5, two at L4, and one at L3. The patients had low back pain that was unimproved after 3-6 months of conservative management. None demonstrated spondylolisthesis or instability, disc degeneration on MRI, nor radiculopathy. All symptoms resolved after isthmus block. The patients were followed-up for a period of 3-27 months(average of 14.8 months). The authors compared pre-operative and post-operative clinical symptoms and radiographic findings retrospectively. Success of the procedure was determined by the following criteria: impro- vement of clinical symptoms(VAS score), absence of isthmic motion on plain radiographs, and evaluation for the presence of a bone mass bridging the transverse processes and laminae using plain radiographs and reconstruction CT scan. RESULTS: All patients demonstrated improvement in pain scores(average of 8.3 to an average of 3.2). A fusion rate of 87.5% was achieved(7 out 8 levels). No complications were noted. The authors were able to demonstrate a potential benefit in using this new technique for the treatment of lumbar spondylolysis. CONCLUSION: Direct repair of spondylolysis with Dynesys spinal system is technically easy and fixes strongly biome- chanically.

ABSTRACT

[Objective]To assess the clinical outcome of the clinical application of TSRH hook screw fixation after direct repair of lumbar spondylolysis in young patients.[Method]From 2002.7~2003.10,12 young male patients suffered from lumbarspondylolysis were selected to be treated with TSRH hook screw fixation after direct repair of the defect.MacNab criteria was used to assess their pre-and postoperative status.Postoperatively,radiographic finding and clinical results were observed.[Result]All patients were followed-up after surgery,ranging from 6 months to 19 months.Radiographs showed all patients having bilateral union;a11 patients with low back pain or radicular pain experienced significantly relieved.Neither complications nor instrumentation failure were observed.[Conclusion]This technique proves to be well-designed and easy to handle,and is considered useful and recommended for direct repair of the defects found in lumbar spondylolysis in young patients.

ABSTRACT

OBJECTIVE: The authors introduce a technique of direct repair of pars defects in symptomatic lumbar spondylolysis using pedicle screws and universal hooks. METHODS: From March 2001 to September 2001, six patients with symptomatic lumbar spondylolysis and positive pars injection test underwent this procedure. Patients with lumbar instability and discogenic pain were excluded. After posterior midline incision, clearing of fibrous, cartilaginous and sclerotic tissues in and around the defects of pars was done until healthy bone was exposed. Strips of cancellous bone were taken from adjacent laminae and packed into the defects of pars. After insertion of the pedicle screws, universal hooks were placed in the lower margin of the lamina of the involved vertebra. With gentle compression between the head of screw and universal hook, the nut was fixed. Same procedures were done on the contralateral side. RESULTS: Postoperatively, all six patients experienced clinical improvement and there was no complication related with operation. CONCLUSION: This new technique is considered useful for direct repair of the defects in symptomatic lumbar spondylolysis without significant injury in the normal structures.

ABSTRACT

PURPOSE: To report our experience of massive rotator cuff tear and evaluate the final outcome and factors influencing the final results. MATERIALS AND METHODS: We have performed 25 repairs (24 patients) of the massive rotator cuff tear. The average age 58 years old, mean follow-up was 22 months. Acromioplasty alone was performed in 7 cases and acromioplasty with direct repair was performed in 18 cases. We statistically analyzed the results by Student's T-test. RESULTS: Postoperative pain and UCLA score improved 2.2, 32 from 7.4, 14 respectively. The gain of 32 degrees in forward flexion, 8 degrees in external rotation could be obtained. Six (24%) was excellent, sixteen (64%) was good and three (12%) was poor and eigthy-four percent of our series was satisfied. Preoperative range of motion (P=0.040) , the degree of degeneration and the magnitude of tendon retraction (P=0.013) influenced the final results, but age (P=0.232) , duration and severity of pain (P=0.370) did not. There was a significant difference between acromioplasty only and acromioplasty with direct repair even though this discrepancy is very small (P=0.046) . Retear was revealed in 8 cases (44%) among repaired rotator cuff. CONCLUSION: Acromioplasty by itself could get a pain relief and functional improvement and a patient satisfaction in the treatment of massive rotator cuff tear regardless a reparability, but a more favorable result could be expected when a torn rotator cuff was repaired if possible.
